Client Registration
Once you have spoken with our team about the process of becoming a new client, you will complete Client Registration forms. These forms are completed online so that we can be as paperless as possible. You are emailed a link to open a client portal where you will start this process. We recommend you complete the registration forms as soon as it is possible so that we have time to review the information well before your initial session.
Couples Registration
As couples counseling requires information from both parties, complete the registration forms together as much as possible so there are balanced input opportunities. After you speak with our team and as with individual registration forms, we will email you the link to open a client portal to complete necessary documents.
Consent to Treat
Notice of Privacy Practices
All new clients will be required to sign a Consent to Treat form as required by HIPAA. This also serves as a written agreement between client and counselor. This will need your (electronic) signature and we strongly recommend you read the Consent to Treat thoroughly and ask questions at the first session or send your new counselor an email with questions beforehand. The Consent to Treat is included in the documents that you will complete via the client portal you will establish after first speaking to your new counselor and are sent a link to be able to do so.
This document describes how personal information about a client may be used and disclosed and how clients may get access to their records. This form will need your (electronic) signature. It is kept in your file showing that you understand HIPAA and that you were provided the information about it.
